    Chorus was formed in 2017 by the merge of three established WA care providers, Care Options, Community First International and Volunteer Task Force. We are an Australian

    not-for-profit organisation providing services in Aged Care, Disability and Mental Health. Together our new organisation will deliver support services to over 10,000 Customers across metropolitan and regional WA with a 500-strong workforce and over 600 volunteers.     A fantastic permanent part-time opportunity has arisen for an individual who is interested in pursuing further their clinical career in customer service within the community care sector. We are interested in hearing from experienced individuals who through the application of nursing skills and knowledge and utilising evidence-based approaches, deliver quality, safe, coordinated clinical care to customers. In addition, you will understand the principles of customer satisfaction and takes personal responsibility to ensure Chorus always delivers a positive customer experience.

     

    About the Role....

     

    Under the supervision of a Team Leader Registered Nurse, the Enrolled Nurse or Registered Nurse role provides quality, competent, clinical care that enhances the well-being of the customer. This will be achieved by understanding the enrolled/registered nurse role and boundaries in relation to scopes of practice within the registered nurse delegation. The role is responsible for ensuring Customers are provided with high quality care throughout the care delivery process whilst embracing customer choice.

     

    Essential Criteria:  

     

    The successful candidate would be able to demonstrate the following attributes:

     

    • Current registration as an Enrolled Nurse/Registered Nurse with AHPRA
    • Minimum of 3 years community experience required
    • Evidence of on-going clinical practice development
    • Advanced wound care/peg and IDC management would be advantageous
    • Demonstrated initiative and ability to work without supervision under the delegation of the Team Leader Registered Nurse
    • Working within their scope of practice and an ability to undertake clinical assessment and reviews for customers is essential
    • Monitor, review and evaluate goals of care for customers
    • Demonstrated computer literacy in word processing, databases, calendar management in order to create effective documentation and correspondence
    • Australian Driver’s Licence
    • Current National Police Clearance with 6 months or less currency
    • Current Provide First Aid Certificate with 12 months or less currency
    • Current CPR Certificate.
    • Understanding of Home Care and Home Care Package programmes will be highly regarded.
